Our hope is to help alleviate some of your financial burdens so that you can focus on your relationships and your life. We understand discussing personal health matters, especially those related to a terminal illness diagnosis, can be a deeply emotional and challenging experience, so we designed this application to help us understand your circumstances without being too difficult to complete. All information will be treated with the utmost respect and confidentiality. There are two portions to this application. The first portion is intended to grant $2,500 in financial assistance. The second portion is intended for a $10,000 grant, and additional financial information is required. Submission of an application does not guarantee approval or receipt of funds. All applications will be reviewed and evaluated based on the criteria set forth by the Foundation. Grant awards are subject to the availability of funds and the Foundation's discretion. Each application will undergo a thorough review by our independent Advisory Committee. The Foundation reserves the right to request additional information or clarification from applicants during the review process.

Apply for a Grant Today

The TriUninty Foundation application is now open for those individuals who are members of St. Cloud Financial Credit Union and Mazuma Credit Union. This application period will close on September 15th, with grants awarded by the end of October.

We understand that filling out one more piece of paperwork can be daunting, so we've designed the application process to accommodate various needs. Here’s how we do it:

  • A short application for assistance of $2,500.
  • A slightly longer application for assistance of $10,000.
Eligibility - In the Event of a Loss
Funding is only available if the applicant is living at the time of distribution. We understand how difficult these moments can be and extend our heartfelt support.

Funding Limits and Guidelines
Applicants may receive multiple grants, but no more than $12,500 total within a calendar year—and no more than $12,500 in their lifetime.  Grants are only approved for the individual diagnosed with a life-threatening illness.  While anyone can help complete the application, funds cannot be awarded to the nominator or family members on their behalf.  

  • $10,000 grants:  At least $7,500 must be paid directly to verified billers.
  • $2,500 grants:    Funds may be paid to the recipient or to a biller.
For minor children, grants are awarded in the child's name and managed by a parent or legal guardian.  If the child doesn't have a bank account, funds can be distributed to the parent or legal guardian, with the expectation they are used to ease the family's financial strain related to the diagnosis.

If the nominee is incapacitated, funds may be distributed to a legally authorized representative, with required proof of authorization. 


Process

1. Register

Click on the "Register" link and complete and submit the registration form.

2. Login

Login to the Grant with your email and password.

3. Create

Create your online application. 

4. Save

During the Call for Applications period, your application can be saved as DRAFT until all the required information is completed and attachments uploaded. As each section is complete, you will see a appear in the category tab when the application is saved. At any time, you can download and print your application by clicking on the icon in the Application Summary section.

5. Submit

On completion, save your Application as FINAL. Download and print a copy of your application for your records by clicking on the icon in the Application Summary section in the right column. Note: If an update is required prior to the Call for Applications period, you can make the update and resave as FINAL.
